At Red Butte Pain Solutions, we’ve treated thousands of people struggling with back pain. Over the years, one type of back pain has consistently challenged both our patients and us: vertebrogenic low back pain. It affects a large percentage of the people we see. For a long time, there wasn’t a reliable option to target the root cause. But that has changed. We’re excited to offer a breakthrough treatment known as the Intracept procedure.

What Is Vertebrogenic Low Back Pain?

Unlike sciatica or herniated discs, vertebrogenic pain originates from the vertebral endplates—the interface between spinal discs and bones. These endplates are rich in nerve supply from the basivertebral nerve (BVN). When endplates become damaged or inflamed, the BVN transmits pain signals to the brain. This condition is especially common in patients who show Modic type 1 or 2 changes on MRI scans.

The hallmark symptom is deep, aching, midline low back pain that worsens with sitting, bending, or transitioning from sitting to standing. There’s no leg pain like in sciatica, just that persistent central back pain that seems to flare unpredictably.

How the Intracept Procedure Works

The Intracept procedure is a minimally invasive, image-guided ablation of the BVN. By targeting and deactivating this nerve, we can significantly reduce or even eliminate the pain signals coming from damaged endplates. Here’s a quick overview of the procedure:

  • The patient lies face down on an X-ray table.
  • A small probe is guided through the pedicle of the vertebra into the center of the bone.
  • Using radiofrequency energy, the BVN is ablated (deactivated).
  • The probe is then removed, and the small incision is closed.

Most patients go home the same day with minimal downtime.

Proven Effectiveness

The Intracept procedure is supported by Level I evidence, the highest tier of clinical research. Key findings include:

  • A major randomized trial showed a 3.8-point reduction in VAS pain scores at 12 months, with 64% of patients experiencing at least 50% relief.
  • At 5 years post-procedure, 66% had more than 50% pain relief, and 34% were completely pain-free.
  • Functional scores (ODI) improved by an average of 25.7 points, with 75% reaching the threshold for meaningful change.
  • Quality of life scores (SF-36) improved by 9–12 points, with sustained benefits over time.

Who Is a Good Candidate?

To benefit from the Intracept procedure, patients must meet these criteria:

  • Chronic low back pain lasting more than 6 months
  • Failed conservative treatments like physical therapy and medications
  • MRI evidence of Modic type 1 or 2 changes at the vertebral endplates
  • No radicular pain, spinal instability, infection, or tumor

Careful patient selection ensures the best possible outcomes.

Safety and Recovery

The Intracept procedure has an excellent safety profile:

  • No reported device-related deaths
  • Minor side effects include temporary soreness, mild numbness, or vasovagal reactions
  • Serious complications (like bleeding or nerve injury) are extremely rare, occurring in less than 0.1% of cases

Most patients resume light activity within a few days and return to full activity in 1 to 2 weeks.

Long-Term Relief Without Surgery

Unlike injections or medications that provide only short-term relief, the Intracept procedure offers:

  • 5+ years of sustained pain relief
  • Fewer flare-ups
  • Decreased reliance on medications
  • Improved mobility and quality of life

What Patients Say

In real-world settings, patient satisfaction matches clinical trial results. Up to 93% of patients report being satisfied with the Intracept procedure. Patients say they feel more mobile, sleep better, and use fewer medications. Many are able to return to work, hobbies, and everyday activities.

A Great Option for Older Adults

Elderly patients with vertebrogenic low back pain can also benefit. We evaluate bone quality using DEXA or CT scans to ensure safety. As long as Modic changes are present and the vertebrae are structurally sound, older adults can achieve excellent results.

Frequently Asked Questions (FAQ)

How long does the Intracept procedure take?
About 60 to 90 minutes.

Will I be asleep during the procedure?
Most patients receive light sedation and local anesthesia. You’ll be comfortable and relaxed.

How long is the recovery time?
Light activity is fine within a few days. Most return to normal life in 1 to 2 weeks.

Is this procedure covered by insurance?
Yes. Most major insurers, including Medicare, cover the Intracept procedure when appropriate, though it may take some work going through the authorization process.

Can it be repeated if needed?
Repeat treatment is rarely needed. Most patients get long-term relief from a single procedure.

How will I know if I’m a candidate?
We’ll perform a detailed evaluation and review your MRI to check for Modic changes.
